Google has implemented a very smooth process to request reissue of a returned or lost AdSense cheque. All you have to do is, visit the following URL and request a reissue of the cheque:

You have to provide the following information when you request reissue of the AdSense payment:
1. Your name
2. Exact amount to be paid as per AdSense reports
3. Reason for request of reissue. (If it was returned by courier service, you may mention that).
